Doctor's Order

Before we can book your initial assessment, we will need an order for outpatient PT/OT/ST from your doctor.

You can email the order to us at info@synapserehab.com OR have your doctor fax the order directly to our office 404-549-2877. 

Initial Contact Call

Once we receive the order from your physician, we will contact you.

During this initial call we will collect any required personal/medical information, verify your insurance benefits, discuss your scheduling preferences, and book your evaluation with one of our expert neurologic clinicians.​

Complete Intake Forms

After the evaluation is booked, you will receive a digital intake form (via email and/or text) to complete prior to your first appointment.
