How does this work?

​

To get started, we need a physician's order stating your diagnosis and "physical therapy and occupational therapy evaluate and treat" faxed to (404)549-2877 or emailed to info@synapserehab.com.

After we receive the order, we will call you to schedule either an in-person or home visit.

 

*Please let us know you prefer to be seen in your home.*

We offer home visits within a 10 mile radius of our Sandy Springs clinic.
